How to fill forms for brokering activity, transit and transportation of defence related products?
 
• Application for registration for brokering activity of defence related products
With that application you may apply for registration for brokering activity of defence related products. It consists of tables with gray and white boxes. The information required in the indicative (gray) fields should be filled in the white boxes of the document.
I. Applicant – describes the type of applicant and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.)
II. Representative of the applicant - describes the name, position, etc. of the applicant’s authorized representative who will submit the application (from the list of authorized representatives of the company), if available.
III. List of attached documents - describes number and date of issue of the attached documents.
IV. Additional information - indicative field where the applicant can describe the additional information.
V. Declarative part – the applicant fills his name, position, signature and stamp.

• Application for licence for brokering transaction, transit licence and transportation licence
With that application you may apply for licence for brokering transaction, transit licence and transportation licence of defence related products. It consists of tables with gray and white boxes. The information required in the indicative (gray) fields should be filled in the white boxes of the document.
1. Applicant – describes the applicant and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
2. Terms of delivery – describes the trilateral code of the terms of delivery according to the International Commercial terms (INCOTERMS).
3. Mode of transport – bilateral code of the modes of transport.
4. Route of movement of items – describes the route of movement of the items.
5. Stated end-use – detailed description of the end-use of the items.
6. Country of final destination – describes the country of the final end-use/end-user.
7. Country of consignee/importer – describes the country of the consignee/importer.
8. Country of the current location of the items – describes the country of the current location of the items.
9. Customs point of entry into the territory of the European Union – it should be filled only in case of transit of defence-related products.
10. Country of origin – describes the country/countries where the items are made.
11. Customs office – describes the customs office where the customs procedure starts. In the „Planned date of delivery” should be filled the expected date of delivery.
12. Exporter/consigner - describes the exporter/consigner and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
13. Importer/consignee - describes the importer/consignee and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
14. End-user - describes the end-user and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
15. Manufacturer - describes the manufacturer and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.). In some cases, where there are more than one manufacturer, the data should be filled in section B1.
16. Other parties involved - describes the other parties involved in the transaction and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.). In some cases, where there are more other parties involved, the data should be filled in section B2.
17. Representative of the applicant - describes the name, position, etc. of the applicant’s authorized representative who will submit the application (from the list of authorized representatives of the company), if available.
18. Description of the items – detailed description of the items.
19-22. The check-boxes should be filled with „õ” for answer „yes” or „no”, according to the purposes of the items-use.
23. Declarative part, where the applicant confirms the truth of the data declared in the application.
24. Number and date of preceding licence related to the items listed in box 18 – describes the number and date of preceding licence related to the items, if available.
25. Additional information - indicative field where the applicant can describe the additional information, related to the current case.
26. List of attached documents – describes number and date of issue of the attached documents.
27. Applicant – the applicant fills his name, position, signature and stamp.

• Licence for brokering transaction of defence related products
It consists of tables with gray and white boxes. The information required in the indicative (gray) fields should be filled in the white boxes of the document.
1. Broker - describes the broker and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
2, 3, 4 è 6 – these fields are being filled by the issuing authority.
5. Exporter - describes the exporter and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
7. Importer - describes the importer and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
8. Country of origin – describes the country/countries where the items are made.
9. Country of consignment – describes the country of the importer.
10. End-user - describes the end-user and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
11. Country of final destination – describes the country of the final end-use/end-user.
12. Description of the items – detailed description of the items.
13. Contract date – describes the number and date of the foreign trade contract.
14. Additional information – indicative field, where the applicant can describe the additional information.

• Licence for transit of defence related products
It consists of tables with gray and white boxes. The information required in the indicative (gray) fields should be filled in the white boxes of the document.
1. Exporter - describes the exporter and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
2, 3, 4, 6 è 14 – these fields are being filled by the issuing authority.
5. Importer - describes the importer and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
7. Representative of the applicant - describes the name, position, etc. of the applicant’s authorized representative who will submit the application.
8. Country of origin – describes the country/countries where the items are made.
9. Country of consignment – describes the country of the importer.
10. End-user - describes the end-user and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
11. Country of final destination – describes the country of the final end-use/end-user.

• Licence for transport of defence related products
It consists of tables with gray and white boxes. The information required in the indicative (gray) fields should be filled in the white boxes of the document.
1. Exporter - describes the exporter and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
2, 3, 4, 6 è 14 – these fields are being filled by the issuing authority.
5. Importer - describes the importer and complete information as required under the indicative fields (name, commercial reg. ¹/ ID ¹, phone, address etc.).
